To encourage nurses to serve as Clinical Nurse Preceptors by providing a partial exemption from tuition to them and/or their children.

Students who’s dependency has changed since completing their FAFSA.

Students complete to allow specific persons to have access to the student’s records.

Used when students want to file a financial aid appeal. Students must complete and submit these forms online, via email, or in person.

To encourage persons employed as fire fighters or active members of volunteer fire departments to take college courses designed to help them in their work.

Students who intend to apply for loans (private loans) other than Federal Direct Loans.

Student must submit and visit with a Financial Aid counselor before being awarded a student loan.

To be completed and signed by student making changes to their financial aid award.

Completed by parent to request a Federal Parent PLUS Loan.

When a student has a past due balance from a previous semester you can fill this form out. Contact the Financial Aid Office to see which option on the form you will need to fill out.

Submitted when student has out-of-pocket expenses to request a reimbursement.

An Affidavit to determine Texas Residency can be submitted to the Admissions office. Once processed, if you qualify for in state tuition you can provide the below documents to Financial Aid to qualify for Texas grants. -a copy of your birth certificate -your TASFA application -IRS tax transcripts or proof of non filing -Eligibility statement which can be found on our Forms page

Students must submit if selected to verify their enrollment history.

Student completes when eligible to work on campus. (Federal and State Work-Study Programs)

Starting with the 24-25 school year, this form must be completed for students to be eligible for any type of state funded grants or loans.

Starting with the 24-25 school year, students must complete this form anytime they want us to release FTI (Federal Tax Information) from their FAFSA to a specified 3rd party.
Downloadable Verification Forms
Below are our Verification Forms available for you to download. Please upload into your MyViking, mail, fax, email or hand deliver completed forms to the Office of Financial Aid, unless otherwise advised on the form. (Link to a PDF file requires Adobe Reader.) Please Note the V4 and V5 forms must be notarized or signed in the Financial Aid Office, because of this the form is downloadable but not fillable.

Students selected for the Standard Verification need to verify their household size and tax information they listed on their FAFSA. Dependent students must supply parent information as well as their own.

Students selected for the Custom Verification will need to verify their identity and high school completion status. The Custom Verification will need to be signed in front of a Notary or in the Financial Aid Office and supply the office with a copy of a valid government issued ID such as their drivers license or passport.

Students selected for the Aggregate Verification Will need to verify their identity and high school completion status as well as their household size and tax information they listed on their FAFSA. For dependent students they must supply parent tax information as well as their own. The Aggregate Verification will need to be signed in front of a Notary or in the Financial Aid Office and supply the office with a copy of a valid government issued ID such as their drivers license or passport.
